We are seeking a per diem LPN (Licensed Practical Nurses) to join our team.  We have an immediate need for an LPN to work as part of a clinical team in providing direct nursing care to medically frail individuals with disabilities. Full Job Description

** Sign-on Bonus of $1,000 - New Starting Rates, and New Differentials**

• On-call
• Weekend availability

Responsibilities:

• Administer medications and implement nursing treatment plans
• Communicate with/provide necessary information on relevant health issues to parents, guardians, and applicable Agency staff
• Hands-on nursing care for children with physical and developmental disabilities (including, but not limited to wound care, catheters, and g-tubes)
• Review patients’ physical condition and medical needs and document changes as appropriate
• Provide guidance and support to direct care staff working alongside LPN

Qualifications:

• Graduate from an accredited nursing program
• Current LPN license to practice nursing in the State of MA
• Strong assessment and documentation skills
• 1-year practical nursing experience a plus (new graduates are welcome to apply)
• Experience with developmental disabilities: 1 year
• Proficiency in delegated nursing skills
• Valid driver’s license
